Tax Prep By Mail Questionnaire

We have two questionnaires: long and short. Most clients will need the long version.

Long (1040/1040A)

If you have any income, adjustments, deductions, or credits beyond those listed below, please use the long version.

Short (1040EZ). Use the short version only if all of the following are true:

  • Your filing status is single or married filing jointly. (Not sure? Click here.)
  • You (and your spouse if filing jointly) were under 65 and not blind at the end of 2009.
  • You do not claim any dependents.
  • You do not claim any adjustments to income.
  • You do not itemize deductions.
  • Your only income came from:
    • Wages, salaries, and tips1
    • Taxable scholarships or fellowship grants
    • Unemployment compensation
    • Alaska Permanent Fund dividends, and
    • Taxable interest of $1500 or less
  • Your taxable income2 is less than $100,000.
  • The only tax credits you may claim are the earned income credit (EIC) and the making work pay credit. (You do not need a qualifying child to claim the EIC, but there are age, income, and other limits.)
  • You did not receive any advance EIC payments.
  • You do not owe any household employment taxes on wages paid to a household employee.
  • You are not a debtor in a Chapter 11 bankruptcy case filed after October 16, 2005.
  • You do not owe any alternative minimum tax.
1If you received tips, including allocated tips, that are not shown in box 5 or 7 of your W-2, you might not be able to use Form 1040EZ.
2To estimate your taxable income as a 1040EZ filer, combine your income from the five sources listed and subtract $9,350 if single, $18,700 if married filing jointly.
